Accessible Parking Permits are issued by the Issuing Agent (Clerk) where you reside.  If you live in the TOWN of Lansing, please go to the Town Clerk's Office.  If you live in the VILLAGE of Lansing, please go to the Village Clerk's Office.  Please take the time to look up your physical residence address and determine where you need to go.  Even if you have an "Ithaca" mailing address, you might not reside in Ithaca (that's just the post office that handles your mail).  If you aren't sure, please call us at 607-533-4142 and we will verify which office you need to go to.  Do NOT take the form to the DMV (Department of Motor Vehicles).

To download a form visit the NY DMV website.  You may also pick up a form at our office or call 607-533-4142 and we will be happy to mail one to you.  When we issue the permit, we are required to see the applicants NYS Drivers License as the last three numbers of the license will appear on the permit.

  • Part one of the application is completed by the person applying for a permit.
  • Part two of the application must be completed only by a medical professional.
    • Permanent Disabilities may be certified (signed) by a Medical Doctor (MD), Doctor of Osteopathy (DO), Physician Assistant (PA), Nurse Practitioner (NP), a Doctor of Podiatric Medicine (DPM, for disabilities related to the foot), or Optometrist (OD, for blindness). 
    • Temporary Disabilities , however, may be certified (signed) ONLY by a Medical Doctor or Doctor of Osteopathy.  

Permanent (Blue) Parking Permits are good for up to five years and are renewable. A new application form is not required.

Temporary (Red) Parking Permits need an expected recovery date and are not renewable.

Accessible Parking Permits are good in any vehicle the person they are issued to is driving or riding in.

There are no fees for an accessible parking permit.
